Doctor's order/referral

Must have prescription from physician (M.D., D.O., P.A., or N.P.) and be medically cleared to be evaluated behind-the-wheel. Once the signed referral form is received, H&T Driver Rehabilitation Specialists, LLC will contact the client and schedule the driver evaluation.

Eye Specialist form (DL-63)

Must be completed by eye specialist (ophthalmologist or optometrist) if client does not meet acuity or peripheral vision standards, or if there is concern pertaining to client's vision.

Physican form for DPS (DL-177)

Must be completed by your physician at the end of driver's training in order for you to take the road test at TEXAS DPS.
