COGNITVE SURVEY

This survey and exercise use program was designed by an Occupational Therapy student as part of her doctorate program. This survey will NOT impact your ability to receive services or be used to determine your ability to drive. This is strictly voluntary for a research project. Thank you for your participation.

In addition, we have included a cognitive training guide. Please follow the training guide program, as this will help our research student better understand if certain cognitive activities can help improve an individual's potential to return to driving.
